I queued outside the Centro de saude on Monday until the cold and rain got to me. I filled in their ‘contact us’ E mail enquirer but have not hade a reply. Any ideas what I might do with this document? Surely it is not necessary to make a doctors appointment?
Please pardon my ignorance but what's a S1 form?

It took a few weeks for the Centro de Saude to get back to us when we registered.
